Report a possible matching or display error.
Public records can be complicated, and identity or relationship mappings can be wrong. Corrections improve this site’s interpretation of the source data without rewriting an accurate government record.
What you can report
- Two people with the same name were incorrectly merged.
- A payment appears to be attributed to the wrong doctor.
- An address, specialty, or identity detail is displayed incorrectly.
- A product match appears incorrect.
- A company or affiliate mapping appears incorrect.
- An official source has been revised.
- A source link is broken or points to the wrong record.
What this site can—and cannot—change
We can correct this site’s identity matching, entity resolution, mappings, calculations, wording, and links when evidence supports a correction.
We cannot modify the original CMS, NPPES, Medicare, FDA, or other source record. An accurate public record is not hidden because a doctor, company, or other party asks for it to be removed.
What to include in a correction report
- The doctor’s name and NPI, if the report concerns a doctor profile.
- The exact page URL and the section you believe is wrong.
- What you believe the correct information should be.
- A primary or authoritative source that supports the correction, when available.
- The date you observed the issue.

How reports are reviewed
- Reproduce the page and identify the exact source/mapping decision involved.
- Compare the report with the underlying public source and the site’s current methodology.
- Correct the site if its identity, mapping, aggregation, wording, or link is wrong.
- Keep the underlying public record unchanged if the source itself is accurate.
- Add or update regression coverage when the error reveals a reusable failure mode.
